Fix OSHA 300/301 CSV Date Format Errors

Correct dates changed by Excel or exported in a format the OSHA ITA case-data template does not accept.

Why this happens

Spreadsheet software may display one date while exporting another. Locale-specific day/month order and missing leading zeros are common sources of rework.
